I am using the online survey creator and then embedding the survey on to my website but I want to know how I can embed the jQuery date picker in to my form?
Is this possible via the online survey creator or do you have to use a self-hosted option?

You can use SurveyJS Custom Widgets to add jQuery datepicker to your survey. Here is live sample in plunker of survey with datepicker. Also you can add jQuery datepicker custom widget to SurveyCreator tollbox as shown in this example as self-hosted solution of SurveyCreator
